The purpose of this refactoring is to provide a convenient way to
write JSON independently of VM service, which is excluded in PRODUCT
build mode.

At the moment isolated has a name field which is on the main_port the
only exception is the vm-service isolate.
The name is used just for logging.
The service protocol though uses an extra field called debugger_name,
that at this moment is redundant and is optimized away during
compilation.

- Removes the debugger_name in favor of a modifiable name
- Adds to all the log where the name was logged the main_port (that is
the only real identifier)
- Changes the default format of the isolate name from:

This cl implements an experimental filesystem maintained in the vm, called dart-devfs. This will allow service protocol users to read and write source files while the vm is running. This is needed for the reload support for flutter.

- Use TokenDescriptor instead of intptr_t for all token positions.
- Use TokenDescriptor in raw_object instead of int32_t.
- TokenDescriptor is a POD with an int32_t (this shrinks the size of AST and IR nodes by 32-bits on 64-bit architectures).

This matches the JSON-RPC 2.0 spec:

While this is technically a breaking change, it seems very unlikely that
any consumers were validating this form of the parameter. The
documentation was already inconsistent about whether it was hyphenated,
and the only code likely to be using would be written against the
JSON-RPC 2.0 spec itself, and would reject the old format anyway.
