Paul Berry 7caaf2846d [docs] Improve link for filing breaking change requests.
Previously, the link text `Dart SDK issue tracker` in the breaking
change documentation pointed to the main page of the SDK issue
tracker. The user had to click `New issue` and select the template
`Propose a breaking change (for contributor use only).`.

With this change, the link goes straight to the template
(https://github.com/dart-lang/sdk/issues/new?template=6_breaking_change.yml),
which should streamline things a bit.
