This CL ensures that the actual buffers are *always* made available via

The embedder guarantees to the free kernel buffers it has provided to:

- Dart_LoadLibraryFromKernel
- Dart_SetDartLibrarySourcesKernel
on isolate shutdown.
All other kernel buffers will get a finalizer attached, which ensures the
kernel buffers get freed by the GC once they are no longer referenced:
- Kernel blobs for expression evaluation
- Kernel blobs for Hot-Reload
- Kernel blobs for cc tests

This is a temporary workaround for VM crashes when using observatory
in bytecode modes. Crashes heppen when CollectTokenPositionsFor
tries to access kernel AST (via kernel_offset()) but there isn't one.

This change replaces kernel AST declarations of fields and functions
with bytecode declarations.
Size of dilp files is reduced by 11-12%.
Startup latency:
Time to the first full frame: 1.945s -> 1.687s
FinalizeClass: 554ms -> 277ms
FinishClassLoading: 296ms -> 156ms
There are following regressions in bytecode mode, which will be fixed
in future:
* dart:mirrors are not supported yet (implementation of mirrors relies
on reading kernel AST in certain cases).
As the result, lib_2/mirrors/* tests fail.
* native extensions are not supported yet (annotations on libraries
and classes in AST are cleaned up as they could reference members
which are now removed from AST).
As the result, standalone_2/entrypoints_verification_test test fails.
* language_2/spread_collections/const_error_test/* tests fail
due to https://github.com/dart-lang/sdk/issues/36286.

This change extracts code which reads kernel in order to get
covariance attributes of parameters into a separate function.
This removes code duplication and makes it easier to hook up
reading of covariance attributes from bytecode in future.

TFA is improved to detect if a method was called using dynamic selector,
was ever called not via this, and whether the tear-off was taken.
This information is used to remove parameter type checks in AOT.
Less accurate selector-based analysis, which was used previously to get
this information, is now used only if TFA is disabled.
Also, precompiler is improved to omit method extractors and implicit
closure functions if analysis proved that tear-off is not taken.

In certain cases StreamingFlowGraphBuilder was used not for building
flow graph, but as an advanced kernel reader. This CL extracts all such
functionality from StreamingFlowGraphBuilder and revises all such uses.
StreamingFlowGraphBuilder constructors without FlowGraphBuilder are
removed. After this CL StreamingFlowGraphBuilder is only used to build
flow graph, as intended.

This CL continues refactoring aimed at isolating StreamingFlowGraphBuilder.
StreamingFlowGraphBuilder dependency is removed from classes
* StreamingConstantEvaluator (it still depends on FlowGraphBuilder)
* ConstantHelper
* SimpleExpressionConverter
and several methods in object.cc.
StreamingConstantEvaluator and ConstantHelper are moved from
kernel_binary_flowgraph{.h, .cc} to a new source file
constant_evaluator{.h, .cc}. StreamingConstantEvaluator is renamed
to ConstantEvaluator.
KernelFingerprintHelper and KernelSourceFingerprintHelper are moved to
a new source file kernel_fingerprints{.h, .cc}.
Instances of kernel::FlowGraphBuilder no longer contain back reference
to a StreamingFlowGraphBuilder. In order to drop this circular dependency
TranslateFinallyFinalizers() is moved from FlowGraphBuilder to
StreamingFlowGraphBuilder.

storing them as int32 values, we are unneccesarily bloating the VM heap.
This change keeps the line starts delta encoded in the VM heap as well.
This brings down the core snapshot size by 275KB (but will of course
affect the debug performance.)

Devirtualization optimization now adds metadata to kernel AST instead
of transforming nodes to Direct* ones. The direct call metadata
provides information about checking receiver for null, while
Direct* kernel nodes do not support null checking.
VM's kernel binary loader is extended to extract arbitrary metadata
from kernel binaries and keep it for flow graph builder.
Kernel flow graph builder is extended to take direct call metadata
into account and generate CheckNull/StaticCall instructions
for devirtualized PropertyGet, PropertySet and MethodInvocation nodes.

Before this change, each function and field had its own kernel data blob
in the VM heap. With this change, the entire kernel data of a library is
stored as one single blob in the VM heap. Functions and fields store an
offset which points to the kernel data, specific to them, in that single
blob.
The pointer to the kernel data for a library is saved in two places:
1. With the library objects themselves.
2. With all the patch classes of the library.
3. With the patch classes created during hot reload.

There already exists a class named "Reader" in the "kernel" namespace
which reads bytes from a kernel binary. The class KernelLoader
(KernelReader before this change) creates VM heap objects and populates
them.

Mostly stream kernel_reader, i.e. the code that sets up the libraries,
classes, methods etc.
Mostly because it still takes a "Program" ast node, and looks at the
"Library" ast nodes to get their kernel offset in the binary.
Currently the scripts (containing breakable points etc) are also created
from the ast nodes.
The rest is now streamed.
This also means that more ast visitors could be deleted.

The canonical name table is copied into a typed data array in the VM's
heap. The encoding is the same as in the binary except that the
integer indexes are fixed-size.
Canonical names are now integer indexes instead of objects allocated
in the C++ heap.

Copy the Kernel string offsets into a uint32 array in the VM's heap.
This avoids allocating small string objects with new and avoids having
a table of the canonical strings.
Instead of an offset and a size, strings are now represented as
indexes into the string table in the heap. The start offset of string
N is found at byte offset N*4 because it is a uint32, and the end
offset is found at byte offset (N+1)*4. The strings themselves are
just integer indexes instead of pointers.
In the stream flow graph builder, string access is all random access.
