Heap references always point into R/RW mapping (this simplifies the
marker) and entry points are retargeted into RX.
To simplify implementation we assume that there are no image pages
created - which means we can always go from Instructions to the start of
the page and check where it is dual mapped to.

We are no longer using Dart VM in a setting where this
matters as a security measure and it just complicates
portability for no benefit.
There is an indication that it is causing problems when
running Linux build of Dart VM under Docker on Mac OS X.

This is follow up to bd57548 which switched us to use manual copying
for call back pages on Mac OS and iOS. However these newly allocated
pages need to be created with MAP_JIT flag otherwise OS will kill
us with code signing violation if hardened runtime is enabled.
This can only be observed when the binary is signed that's why
we have not seen it on CI.
TEST=manually signed and tested that it no longer crashes.

The new Secure Page Table Monitor in iOS on A15 and A16 does not allow
`vm_remap`ping memory pages that are manually created. It only allows
remapping pages which come directly from a signed binary.
Manually copying the pages is supported in JIT mode.

This is the same metadata system that will support async callbacks. The
main thing this does is take the trampolines that used to be JIT only,
and use them in AOT too. This is partly a safety thing (there's some
extra checks that used to be skipped on AOT), but mostly just so that
the metadata system is unified between the sync and async callbacks.

Change executable pages to be read/execute but not writable by default.
All pages are made temporarily writable just before a full GC, because both
the mark and sweep phases write to the pages. When allocating in a page and
when patching code, the pages are made temporarily writable.
The order of allocation of Code and Instructions objects is changed so that
a GC will not occur after Instructions is allocated. (A full GC would
render the Instructions unwritable.) A scoped object is used to make memory
protection simpler.

The purpose of this change is twofold:
1. Source in the bin directory can now use the same assertions as
source in the vm directory. The ASSERT macro used by the code
in runtime/bin was just defined to use assert from the standard
C library.
2. Moving other implementation parts from runtime/vm to
runtime/platform (e.g. classes Monitor and Mutex) for sharing
between runtime/bin and runtime/vm will be easier as these
implementations rely on these assertion macros.
Created two gypi files for the platform directory. One for the
headers and one for the source. The source one is only included
when building the VM library and will be present in libdart.a
when the dart executable is linked.
All the code for asserts is still in the dart namespace.
Also re-arranged the order of includes to be alphabetically in
the files touched.
