After previous refactoring, StreamingScopeBuilder no longer depends on
StreamingFlowGraphBuilder.
This CL moves StreamingScopeBuilder to a separate file
vm/compiler/frontend/scope_builder{.h, .cc}.
Also:
* StreamingScopeBuilder is renamed to ScopeBuilder.
* IntMap is moved to hash_map.h.
* Minor cleanup of unused forward declarations and friend classes.

Before this CL the test fails because we land on the "await" when
stepping out of "helper" as "helper" is executed synchronously (i.e.
the call to await (which is translated into a call to an await-helper)
hasn't executed yet so landing on it is "correct").
This CL changes the test so "helper" is not executed synchronously by
adding an "await null;" as the first thing in the test. Once we step out
we thus have already executed the "await" and doesn't land on it.
An alternative fix would have been to pass --no-sync-async.

StreamingScopeBuilder does not build a flow graph, but it was creating
and using StreamingFlowGraphBuilder under the hood (in order to read kernel).
This change makes StreamingScopeBuilder independent of StreamingFlowGraphBuilder
and prepares StreamingScopeBuilder to be moved out of kernel_binary_flowgraph.
Also, several unused classes, fields and parameters are removed.

This change continues refactoring to split kernel_binary_flowgraph and
kernel_to_il and move out parts which have no dependencies on flow graph
construction.
Bytecode reading is moved to a separate file bytecode_reader{.h, .cc}.
Metadata helpers, TypeTranslator, ActiveClass and other Active* are moved
to kernel_translation_helper.
Removed dependency of ICData tags on IL Instruction tags, so ICData objects
can be created by bytecode reader without depending on IL.
Unused AlternativeScriptScope is removed.

a ConnectionTask object that can be cancelled. Cancelling
a ConnectionTask closes any sockets that were opened for the
connection attempt that are not yet connected to the host.
This allows a closing HttpClient to close sockets for pending
requests whose sockets weren't fully connected yet.

The VM debugger expects _completeOnAsyncReturn to be called when
returning from an async function for stepping out of the async function
to work.
This CL does just that.
Note that `_completeOnAsyncReturn` only exists in the VM, but as
the async transformation is only run for the VM target that shouldn't
be a problem.

The stub was marking the thread as executing VM code too early, since the slow
path of AllocateArray (called via PushArrayOfArguments) calls into the runtime.
With this fix, DeltaBlueClosures can run in fully interpreted mode.

Rationale:
Generation-based fuzz testing for Dart that
finds divergences for different execution paths
(e.g. AOT vs JIT) on randomly generated Dart programs.
This project is still under heavy development, this
first CL just introduces the concepts.
Note:
The JS mode may report quite a few divergences due to
dart2js - VM semantic differences on integer ops.

inlining 64-bit and double getter/setter operations.
The runtime over all data types (with and without
view) now no longer has outliers.

This allows optimizer to eliminate redundant code branching on type_args_len
which arises when inlining generic methods.
Start refactoring how loads of native fields are represented: instead of simply
setting a bunch of unrelated attributes introduce NativeFieldDesc structure
that captures attributes of native fields, which don't have representation
as Field objects.
Over the time we should be able to eliminate "raw" LoadField/StoreInstanceField
instructions that just address fields by their offset and use NativeFieldDesc-s
in load forwarding to disambiguate different native fields that happen to
have the same offset.

This CL removes all dependencies on StreamingFlowGraphBuilder from
all metadata helpers and StreamingDartTypeTranslator.
This further decouples parts which are not related to flow graph building
and prepares them for moving.

Throwing a programmer-constructed FallThroughError is not useful. It
prints something like:
'null': Switch case fall-through at line null.
And if you can track that down, it's confusing because it's being used
to signal things that don't have anything to do with falling through
from one switch case to the next one. Usually, there turns out to be
no matching case in the first place so there's no possible way that
anything fell through to the next case.
