- Split the Thread structure to be a pure Dart per thread structure and add
a pointer to os_thread which points to the OSThread structure
- Change Schedule/UnSchedule to set the Dart Thread structure as the TLS of
the thread when it is inside the Dart world and reset the TLS back to the
OSThread strcuture when is exits the Dart World.
- Moved the stack_base and few stack size related functions to OSThread from Isolate

- We had a race where the thread pool would be starting up a thread while Dart::Cleanup was running on the main thread.
- The mutex used to guard the thread list was being deleted and set to NULL.
- Thread would try and add itself to the list and die when attempting to lock the mutex.
This CL does two things:
- Moves Thread list mutex cleanup to only run under FLAG_shutdown.
- Fixes a forgotten NULL initilization of thread list mutex.

Due to the need to reclaim TimelineEventBlocks from the system, it is not safe to hold onto a TimelineEvent other than briefly to fill it out.
- Switch TimelineDurationScope to emit two events (begin, end) instead of one duration event.
- When setting duration arguments, hold them in the TimelineDurationScope and then emit them with the end event.
The above two changes allows TimelineDurationScope to work without holding onto a TimelineEvent.
To stop the block from being reclaimed while we are filling out an event, calls to StartEvent return with a lock held that is unlocked when CompleteEvent is called.

This change add the ability to restart the vm through the service
protocol. All isolates are killed, and then the main isolate is
restarted cooperatively by the embedder.
This change also fixes the message handler to prevent it from
accidentally ignoring vm shutdown messages.
Previously, we would stop handling messages whenever we hit an error
(such as a compile error or an unhandled exception). This would leave
shutdown requests sitting the oob queue, neglected.
We now process *all* oob requests, up to the first shutdown request.
When we hit a shutdown request, we clear the oob queue and process no
more messages.
To make all of this work, we had to change the return value of
HandleMessage from bool to a new enum type, allowing the message
handler to distinguish *normal* error cases from the more rarified
shutdown and restart cases.

Remove typedef hack by templatizing the shared structures,
parameterized by block size.
Smaller marking stack blocks are needed for better work distribution during parallel GC.
As a consequence, MarkingStack and StoreBuffer have separate global caches of empty blocks.
Also, use inheritance to differentiate: only StoreBuffer needs the threshold-checking functionality.

and use it in generated code for allocation.
Change generated allocation code to load allocation-top and

If null is not initialized before the VM Isolate's ObjectStore, the
ObjectStore's fields are initialized to kHeapObjectTag, which causes
a crash in VM Isolate shutdown.
There is also a crash when the VM Isolate is signaled with the
shutdown event, so this CL removes adding a Debugger to the
VM Isolate, so that there is no need to shut it down, and send
the signal.
NB: This change does *not* actually enable shutdown. It just fixes
crashes that would happen if it were enabled.
