[scanner] Don't give the Utf8 scanner a 0-terminated byte sequence
...
Not having to do the read-allocate-copy dance for files to add a 0-byte
at the end results in these changes when using the CFE to compile
(a fixed version of) the CFE:

[js_runtime, js_dev_runtime] Implement microsecond field of DataTime

- Move DateTime implementation for dart2js and DDC into a shared place to reduce duplication.
- Add a _microsecond field to the web DateTime to track microseconds outside of the JavaScript Date.
- The cute dart2js optimization whereby `DateTime.now().millisecondsSinceEpoch` is compiled to `Date.now()` still works.
- Both implementations report better errors.
- Fixed VM bug with in-range sentinel.

[dart2js] Add option to only emit deferred load ID mapping.

Some customers use tools to preprocess this set of load IDs in order to optimize how code is loaded in their app. This flag allows us to emit this information as early as possible in the compiler without having to completely compile to JS.

loading algorithm assumed that in the ConstantCollector visitor. Now that Dart supports them, we need to ignore them if they get encountered.

[dart2js] Avoid adding empty type rules.

The fragment emitter already tries to skip the call to `addRules` if the
ruleset is empty, but this occurs too early. The ruleset encoder strips
tautologies like `T <: T` and `InterfaceType <: Object`, but this occurs
after the empty check.
This CL removes the preprocessing from the encoder and instead performs
checks when entries are added to the ruleset, fixing the issue.

[dart2js] Always compute impacts directly on kernel.

While this adds an intermediary form for impact computation, it did not
seem to noticeably change the time it takes to compute the closed world
on a large application.
In addition, it will be a necessary change if we want to start moving
some of the validation currently being performed while building
`ResolutionImpact`s modularly.

[dart2js] Streamline dependency processing in deferred_load.dart.

This is an extensive refactor of deferred_load.dart. The major changes
are:
* An 'EntityData' class to better model some of the polymorphism in the
algorithm
* An 'AlgorithmState' class to simplify updates.
* An 'EntityDataCollector' to encapsulate collecting dependencies from
'EntityData' objects.
NOTE: This patch may change output file numbering.
