Stephen Adams 1472eba52d js_runtime: Split _asyncHelper into separate functions 
The separate functions have smaller call-sites and less overhead.

_asyncStart uses Future._nullFuture to avoid 1 allocation.

The code point id is meant to be basis for AST/Kernel-IR equivalence and the test framework should thereby be portable to a Kernel based inference.
