Nicholas Shahan 863016caf3 [ddc] Make escapeIdentifier API non-nullable
All uses either already pass a non-nullable String or assert that
the returned value is non-nullable which would be equivalent to
asserting that the value passed is not null.

Nicholas Shahan 95cad9c8a6 [ddc] Fix semantics of capturing loop variables
Fixes the for loop lowering to JavaScript when there are function
expressions in the loop initialization, condition, or updates.

Manually assign source mapping information on js_ast nodes to
make stepping in the Dart debugger make as much sense as possible.
With these mappings three steps are required on each loop variable
on each iteration but it ensures that the mapping always points to the
variable instead of some random line of code in or around the original
for loop.

Fixes the failure on language/loop/for_variable_capture_test.

Adds simple method inlining for select patterns only in the dart:_rti

Chloe Stefantsova de302d7f3b [cfe] Introduce StructuralParameters
This CL introduces StructuralParameter and StructuralParameterType
classes. They are intended to replace TypeParameter and
TypeParameterType respectively where those were used as type
parameters defined by FunctionTypes. Previously, type parameters of
FunctionTypes were represented by TypeParameter objects with the
‘parent’ field set to null. By introducing StructuralParameter and
StructuralParameterType this CL unambiguously separates the two
notions of type parameters.

Remove the special case for `class C = Object with M;`
and fall into the same code path that would be used for
`class C2 = BaseClass with M;`

This introduces one new class to the hierarchy but that
class contains the members synthesized on the class. This
ensures they do not get lost during the mixin application at
runtime.

It appears likely that this has been broken for some time and
generating incorrect code when methods require covariant checks.

Johnni Winther 1aa6f00107 [kernel] Refactor CanonicalName/Reference integration
The CL is a step towards have a more restricted and wellstructured
handled of references and canonical names.

The CL moves Reference to canonical_name.dart and makes
CanonicalName.reference private, and replaces CanonicalName.getReference
with a 'reference' getter.

It also removes NamedNode.canonicalName, Field.getterCanonicalName and
Field.setterCanonicalName so that these can only be accessed through the
corresponding reference. This is to reduce the reliance on the
canonical names which, ideally, should only be part of serialization and
deserialization.

This CL is the sum of the following 5 CLs:
